If you find the “Submit" button does not work, this could be due to compatibility issues with
the PDF viewer or the program used to open the form (e.g., Chrome or Microsoft
Edge). The form works best with Adobe Acrobat Pro or Acrobat Reader, which are specifically designed for filling and submitting PDF forms.
A couple ways around this:
If you are filling in the forms electronically, there are a couple ways to submit them to the school:
- You can save/download the fillable form to your computer/tablet as a PDF. Open it on your computer/tablet, fill in and attach it to an e-mail and send to mloff@westernsd.mb.ca. If you submit the registration forms online, feel free to send a copy or photo of the required documents listed below via e-mail to mloff@westernsd.mb.ca or stop in at the school and we can copy it for you.
- You can fill in the paperwork and print it to drop it off.
We require the following documents when registering your child at school either online or in person:
- a birth certificate or other legal document with legal name and birthdate
- a copy of your child's permanent resident document or student/visitor visa document if you are new to Canada
- proof of address (utility bill, driver's license)
Any families requesting a school outside of their school catchment area (with the exception of French Immersion) will need to fill out a School of Choice form. These are available on our school website under the title “Registration Information" or at the school office.
We will always confirm receipt of your child's registration once received. If you have not received a confirmation email, please contact us.
