If you find the “SUBMIT" button does not work, this could be due to compatibility issues with the PDF viewer or the program used to open the form (e.g., Chrome or Microsoft Edge). The form works best with Adobe Acrobat Pro or Acrobat Reader, which are specifically designed for filling and submitting PDF forms.
You
could also download the form to your device as a PDF, then open it on your device, fill in and attach it to an e-mail and send to mloff@westernsd.mb.ca. If you submit the registration forms online, feel free to send a copy or photo of the required documents listed below via e-mail to mloff@westernsd.mb.ca or stop in at the school and we can copy it for you. We require the following documents when registering your child at school either online or in person:
- a birth certificate or other legal document with legal name and birthdate
- a copy of your child's permanent resident document or student/visitor visa document if you are new to Canada
- proof of address (utility bill, driver's license)
Any families requesting a school outside of their school catchment area (with the exception of French Immersion) will need to fill out a School of Choice form. These are available on our school website under the title “Registration Information" or at the school office.
We will always confirm receipt of your child's registration once received. If you have not received a confirmation email, please contact us.
