If you are submitting any of the forms online, but find the “Submit" button does not work, it may be due the program used on your computer/tablet, etc. for opening PDF files. They are not all compatible (Chrome & Microsoft Edge may not allow the “Submit" function to work). This form works best with Adobe Pro or Acrobat PDF Reader. A couple ways around this:
- You can save the fillable form to your computer/tablet as a PDF. Open it on your computer/tablet, fill in and attach it to an e-mail to mloff@westernsd.mb.ca
- You can save the fillable form to your computer/tablet as a PDF. Open it on your computer/tablet, fill in and the “Submit" button should now work.
- You can fill in the paperwork and print it to drop it off.
